Senior Home Care

At Mercy Care Providers, we strive to provide meaningful in-home care to all our clients by clearly defining our individually tailored care plan to meet the patient’s needs 24/7. Our Registered Nurses create a step by step care plan of different activities to meet the patient’s needs during each work shift. Our staff are trained to clearly communicate with our clients and their families about everything we do. This helps create the much-desired positive mood necessary for patient care.

We strive to provide the best care possible by personalizing our patient’s care. We achieve this very unique goal by continuously monitoring our clients and ensuring ongoing communication. All of our home care services are designed to be provided to our clients wherever they reside and require our assistance. It could be a private residence, rehabilitation center, an assisted living facility, senior apartment complex, nursing home, or even an independent living setting.

At Mercy Care Providers, we believe senior care should empower, not restrict. Our thoughtful in-home services are tailored to each individual so they can continue enjoying life’s simple joys from the comfort of home. Whether you need a hand with daily tasks, nursing care after a hospital stay, or just friendly visits, we’re here to help.

Custom Care Plans to Meet Your Needs

We take the time to thoroughly get to know each client so we can create a personalized care plan that feels like it was designed just for them. Our team of experienced Registered Nurses will sit down with you and your loved one to define a step-by-step care plan outlining activities tailored specifically to the patient’s unique needs.

The care plan will outline the types of assistance needed, preferred daily routine, hobbies and interests to engage in, dietary needs and preferences, and any other considerations to ensure your loved one’s values and personality shine through the care provided. We’ll also discuss communication preferences, such as how often you’d like updates on your loved one’s well-being.

As your loved one’s abilities, preferences and health needs inevitably evolve over time, we will collaboratively modify the care plan while keeping open communication with families every step of the way.

We will conduct regular care plan reviews, checking in at least monthly, to proactively identify any changes in condition or emerging needs. This ensures we are making appropriate adjustments to the care provided so it continues meeting your loved one’s needs as they change. You’ll have peace of mind knowing the care adapts right along with any new challenges.

Encouraging Independence in a Secure Home

While our caregivers are there to lend a helping hand with any daily living tasks that have become difficult, our priority is to encourage clients to perform as much as they comfortably can on their own. We want to nurture what they “can” do, not take over what they “can’t”.

For example, our caregivers may provide stand assist to help a senior who is shaky walking on their own, but allow them to complete as much of the walking as possible. Or we may lay out an outfit for someone who struggles dressing themselves, but let them put on the clothes themselves. With grooming, we may help wash difficult to reach spots, but have the senior do as much as they can. We empower seniors to retain their dignity through independence.

Of course, safety remains our top concern. Our team is always vigilant in identifying and addressing any potential fall risks such as tripping hazards. We will install grab bars, railings, ramps or other assistive equipment as needed. We’ll discuss any safety concerns with families and collaborate on solutions. You can trust that your loved one is in the most secure environment possible.

Supporting Family Caregivers Too

Bringing care into the home not only provides comfort for your loved one, but also much-needed support for you as the family caregiver. We understand how physically and emotionally draining hands-on caregiving can be for family members, especially while trying to balance other responsibilities. That’s why our services are designed to uplift not just your loved one, but you as the caregiver too.

Whether it’s providing a break so you can focus on yourself, lending an empathetic ear when you’re feeling overwhelmed, or providing guidance on care techniques, resources, and managing stress, our team is here for the entire family. Caring for a loved one together with support rather than trying to do everything alone makes all the difference in maintaining caregiver health and preventing burnout.

We encourage families to share insights on their loved one’s history, personality, likes and dislikes, and any concerns so we can provide the most tailored, loving care possible. As outside caregivers, we want to partner with families to wrap the senior in care while still maintaining the familial bonds that bring joy and purpose.

Additional Services

In addition to our standard hourly and live-in home care services, we also provide specialized offerings such as:

  • Skilled Nursing Care – Our registered nurses can provide more advanced medical services right in the home setting such as wound care, injections, disease management, and postoperative care. This is ideal for seniors who need continued nursing oversight and care coordination after a hospital or rehab discharge. Our RNs seamlessly bridge the transition home.
  • Dementia Care – Our caregivers are specially trained in validation techniques, communication approaches, activity strategies and safety measures needed to provide empathetic care for seniors with Alzheimer’s or other dementias. We help create a soothing, engaging environment catered to their needs.
  • Respite Care – We give primary family caregivers a much-needed break by providing temporary hourly or overnight care on occasions when you need to travel, attend to your own health or just recharge. Let us shoulder the care while you focus on yourself – it’s essential for maintaining your own wellbeing as a caregiver.

No matter your loved one’s needs or situation, we likely have a service to help make life easier, safer and more joyful. Please ask us how we can support you.
